Job summary

The University of Illinois Hospital & Clinics (UI Health) seeks a full-time Advanced Practice Provider (APP) for transplant outpatient care. The APP-APRNCNP or PA-C will join a multidisciplinary team to deliver high-quality, cost-effective care across all ages in family medicine scope.

Minimum qualifications include APRN/PA training, national certification, Illinois licensure, BLS, and DEA. Travel to Libertyville and Gurnee clinics may be required as part of duties.

Qualifications

• Graduation from an APRN or PA training program.
• Board Certification from ANCC or NCCPA is required.
• Current Illinois license as APRN-CNP or PA-C.
• BLS certification is required.
• DEA license and ability to meet medical staff privileges.

Responsibilities

• Perform history and physical examinations and develop care plans.
• Prescribe medications and therapies within scope of practice.
• Coordinate care with multidisciplinary team and supervise clinical activities.
• Travel to offsite locations as needed (Libertyville, Gurnee).
