Job summary

Baylor St. Luke’s Medical Center in Houston, TX seeks an RN Transplant Coordinator to guide patients from referral through recovery, coordinating evaluation, testing, and in-hospital care.

The role requires a BSN, TX RN license, and at least 3 years of acute care/ICU or transplant experience, with strong communication and advocacy skills to support patients and families throughout the transplant journey.

Qualifications

  • Bachelor of Science in Nursing required at hire.
  • Three years of experience required at hire.
  • Acute Care/ICU and/or transplant experience required at hire.
  • Registered Nurse license in TX required at hire.

Responsibilities

  • Coordinate referral and evaluation for potential transplant candidates and prepare data for Medical Review Board.
  • Manage in-hospital patient care and multidisciplinary rounds to streamline post-op courses.
  • Communicate with referring physicians and payers to reduce denials and ensure updates.
  • Lead pre-op and post-op education and develop teaching plans for patient learning needs.
  • Support staff and community education in transplant and organ donation, and participate in research and publications.

Job Summary and Responsibilities

As an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.

Every day, you will expertly manage the full spectrum of transplant coordination: from initiating referrals and evaluating candidates, to leading in-hospital patient care, managing education, facilitating communication with all stakeholders including payers, and maintaining critical data.

To be successful in this role, you will combine advanced clinical expertise with exceptional organizational and communication skills, a meticulous eye for detail, and an unwavering commitment to patient advocacy, driving both individual success and program excellence.

  • Initiate the referral and evaluation process for potential transplant candidates. Review medical information, schedule testing including physician and coordinator consultations, collect data, review results and compile data for presentation of patient\'s case at Medical Review Board.
  • Primary responsibility for the day-to-day course of in-hospital patients. Notify consultants, prepare for and participate in physician-led and multidisciplinary rounds, and communicate with patients, families and all disciplines involved in patient management in order to streamline post-op course and identify and solve potential problems.
  • Maintain effective communication with referring physicians and third party payers. Provide clinical updates to payers in order to eliminate denials.
  • Initiate and oversee all pre-op and post-op education, collaborating with nursing staff to provide a comprehensive teaching plan. Assess potential learning problems (language barrier, etc.) and structure teaching sessions to meet individual patient needs.
  • Provide staff and community education in the areas of transplant and organ donation.
  • Initiate and collaborate in research protocols, journal articles, abstracts and patient education material.
