Job summary

CommonSpirit Health is seeking an RN Transplant Coordinator in Houston. You will manage the referral and evaluation process, coordinate in-hospital patient care, and oversee education for patients, families, and staff across the transplant program.

You will collaborate with physicians, payers, and multidisciplinary teams to optimize patient outcomes, ensure smooth post-op care, and advance program goals within a large health system.

Responsibilities

  • Initiate referrals and evaluation for transplant candidates.
  • Lead day-to-day in-hospital patient care and rounds coordination.
  • Provide updates to payers and manage communications with all stakeholders.
  • Oversee pre-op and post-op education with nursing staff.
  • Deliver staff and community education on transplant and organ donation.
  • Contribute to research protocols and education materials.

Job Summary and Responsibilities

As an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.

Every day, you will expertly manage the full spectrum of transplant coordination: from initiating referrals and evaluating candidates, to leading in-hospital patient care, managing education, facilitating communication with all stakeholders including payers, and maintaining critical data.

To be successful in this role, you will combine advanced clinical expertise with exceptional organizational and communication skills, a meticulous eye for detail, and an unwavering commitment to patient advocacy, driving both individual success and program excellence.

  • Initiate the referral and evaluation process for potential transplant candidates. Review medical information, schedule testing including physician and coordinator consultations, collect data, review results and compile data for presentation of patient\'s case at Medical Review Board.
  • Primary responsibility for the day-to-day course of in-hospital patients. Notify consultants, prepare for and participate in physician-led and multidisciplinary rounds, and communicate with patients, families and all disciplines involved in patient management in order to streamline post-op course and identify and solve potential problems.
  • Maintain effective communication with referring physicians and third party payers. Provide clinical updates to payers in order to eliminate denials.
  • Initiate and oversee all pre-op and post-op education, collaborating with nursing staff to provide a comprehensive teaching plan. Assess potential learning problems (language barrier, etc.) and structure teaching sessions to meet individual patient needs.
  • Provide staff and community education in the areas of transplant and organ donation.
  • Initiate and collaborate in research protocols, journal articles, abstracts and patient education material.
