Job summary

Baylor St. Lukes Medical Center in Houston seeks a Ventricular Assist Coordinator RN who provides specialized nursing care for patients with advanced heart failure requiring mechanical circulatory support or bridging to transplant.

You will coordinate pre/post-implant care, educate patients and families, collaborate with multidisciplinary teams, monitor for complications, and ensure timely approvals and documentation for device therapies.

Qualifications

  • Bachelor of Nursing (BSN) and 5+ years ICU or transplant experience on hire.
  • Active Registered Nurse license in TX on hire.
  • Basic Life Support (CPR) certification within 14 days of start.

Responsibilities

  • Collaborate with physicians and care teams to plan pre- and post-implant care for patients with mechanical circulatory support.
  • Serve as primary daily care coordinator for in-hospital patients; coordinate rounds, communicate with patients, families and other disciplines to streamline care.
  • Educate patients and families about mechanical circulatory support and post-implant care.
  • Provide clinical updates to payers and ensure pre-certification approvals for procedures.
  • Lead healthcare team with case managers, nurses, physicians, social workers to optimize cost and quality of care.
  • Collaborate with physicians and fellows on treatment strategies for heart failure management.

Job Summary and Responsibilities

As our Ventricular Assist Coordinator RN, you will provide specialized nursing care centered on the medical and surgical care of patients with advanced heart failure who are potential candidates for mechanical circulatory support, all patients who are on advanced forms of mechanical circulatory support, and/or are being bridged to a heart transplant.

Every day, you will play a critical role in managing these complex patients, from initial assessment and education through ongoing support and coordination of their advanced therapies. This involves meticulous care planning, patient and family education, collaboration with multidisciplinary teams, and vigilant monitoring to ensure optimal outcomes for highly vulnerable individuals.

To be successful in your role, you will strategically coordinate comprehensive nursing care for patients with advanced heart failure, particularly those on or being considered for mechanical circulatory support or heart transplant. You will demonstrate exceptional clinical expertise, provide specialized support, and manage complex care needs with precision, ensuring optimal outcomes for a critically ill patient population throughout their unique continuum of care.

  • Collaborate with physicians and other members of the health care team to plan and render pre and post-implant care for the patient with mechanical circulatory support
  • Responsible for the primary day-to-day course of in-hospital patients. Notify consultants, prepare for and participate in physician-led and multidisciplinary rounds, communicate with patients, families and all disciplines involved in patient management in order to streamline hospital course, and identify and solve potential problems
  • Evaluate patient’s learning abilities, develop and prepare educational materials, and provide teaching and education to patient and their family regarding the need for mechanical circulatory support and the care needed post-implant
  • Accountable for maintaining effective communication with referring physicians and third party payers. Provide clinical updates to payers in order to eliminate denials and ensure that all procedures, etc. receive pre-certification approval
  • Accountable for leading the healthcare team with expert advice to case managers, nurses, physicians, social workers and other health-care members as appropriate Identify and Responsible for cost and quality issues related to patient care to ensure resources are being utilized appropriately and in the most cost-efficient manner while maintaining a high level of patient care
  • Collaborate with physicians and Fellows in the development and implementation of strategies unique to the treatment of patients with heart failure
Job Requirements

Required

  • Bachelors Of Nursing BSN and Five (5) years of related work experience in ICU or Transplant, upon hire
  • Registered Nurse: TX, upon hire and
