Job summary

Advocate Health’s Hematopoietic Transplant and Cellular Therapy Clinical Pharmacist role is based in inpatient and clinic settings with a Monday–Friday 0800–1630 schedule, weekend rotations about every 8 weeks, and on-call duties about two weeks. A sign-on bonus is available for eligible candidates, with licensure and education requirements as listed.

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care, including literature review and design of medication therapy regimens.
• Provides patient counseling on medications, adverse effects, compliance, and administration.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens per literature and policies.
• Reviews and interprets medication orders and prescriptions accurately.
• Participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities.
• Manages formulary adherence and assists in obtaining non-formulary/compassionate/investigational meds.
• Ensures compliance with organizational policies and regulations; oversees storage and handling of meds.
• Serves as a preceptor and mentors pharmacy staff and learners.

Education

Bachelor’s or Doctorate degree in pharmacy (ACPE-accredited) or FPGEC NABP certificate
