Job summary

UNC Health Care System, located in Chapel Hill, NC, seeks a transplant case manager/social worker to support pre- and post-transplant care within the Kidney Pre Transplant Program. Qualifications

• Master’s degree in Social Work required for role.
• Licensed Clinical Social Worker (LCSW) in NC; CCTSW encouraged within three years.
• Minimum three years of relevant experience; transplant social work preferred.

Responsibilities

• Screen and assess for transplant and case management needs per CCM guidelines.
• Educate patients and families about pre-, peri-, and post-transplant processes.
• Develop culturally sensitive, evidence-based patient plans and discharge plans.
• Coordinate care across multidisciplinary teams and with payers for authorization.
• Ensure regulatory compliance with CMS, OPTN, TJC, and NC Transplant regs.
