Job summary

Sentara Heart Hospital in Norfolk, VA seeks a Transplant Coordinator RN for the Advanced Heart Program to orchestrate the complex logistics of heart transplantation while prioritizing patient safety and donor/recipient collaboration.

The role manages transplant/VAD evaluation, process, and follow-up care, providing clinical leadership to the care team to plan, direct, manage outcomes and evaluate care. A BSN and RN licensure are required; CCTC is required within three years.

Qualifications

• BSN is required.
• Virginia RN license or multi-state compact eligible.
• BLS certification within 90 days of hire.
• CCTC certification required within three years of hire.

Responsibilities

• Coordinate the clinical management of transplant/VAD patients including evaluation, process, and follow-up care.
• Provide clinical leadership to care team members to plan, direct, manage outcomes and evaluate care.
