Job summary

Northwestern Medicine is seeking a Transplant Nurse Coordinator - PNE. A Registered Nurse with a BSN and IL licensure supports stem cell or organ transplantation coordination under transplant physicians, guiding patients through all care phases, coordinating referrals, tests, and follow-up, and educating patients and families.

The role emphasizes collaboration with the interdisciplinary team, adherence to policies and accreditation standards, and participation in quality improvement initiatives

Qualifications

• BSN required.
• RN license in Illinois (IL).
• BLS certification required.
• 2-3 years of recent & related clinical experience.

Responsibilities

• Plan, direct, provide, and document appropriate patient care.
• Coordinate comprehensive care including consent, data collection, follow-up, referrals, testing and treatment.
• Collaborate with interdisciplinary team to maximize wellness and patient outcomes.
• Educate patients, families, and staff; serve as a resource for ongoing education.
• Act as a communication link between patient, family, team, and outside facilities.
• Contribute to quality improvement initiatives and adherence to policies and regulatory standards.
