Job summary

Mount Sinai is seeking a Transplant Coordinator (RN/NP/PA) to coordinate clinical programs within the Transplantation Institute. You will oversee screening, evaluation, lab work, waiting list decisions, and education for transplant candidates.

In this role, you will work with surgeons, hepatologists, nephrologists, social workers, nutritionists, pharmacists, and finance teams, including after-hours calls and on-call organ offers. A NY license is required; MS/NP preferred.

Qualifications

• Baccalaureate Degree in Nursing is required.
• Masters in Nursing and certification in specialty area are preferred.
• Or: Baccalaureate Degree in Physician Assistant is required; Masters preferred.
• New York State Nurse/NP/PA License with current registration.

Responsibilities

• Coordinate multidisciplinary screening and evaluation of transplant patients with surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance.
• Ensure lab tests are performed and results updated (including tissue typing with Rogosin).
• Educate patients on transplant options, waiting list status, and immunosuppressive regimens.
• Communicate with active transplant candidates about waiting list status and clearances.
• On-call duties include coordinating organ offers and responding to after-hours inquiries.
