Job summary

OneLegacy in California is seeking a Procurement Transplant Coordinator I to manage the organ procurement process within hospitals and recovery centers. You will identify referrals, coordinate care, document procedures, and support families throughout donation.

As a clinical professional, you will work with transplant teams to optimize organ function, ensure compliance with policies, and provide education to hospital staff while traveling within the donor service area.

Responsibilities

• Responds to donor referrals and assesses suitability.
• Establishes relationships with hospital staff to increase referral activity.
• Collaborates with RTS, CRS, SOP, Medical Director and clinical leadership to manage active referrals.
• Reviews hospital medical records and transcribes information to digitalDONOR per policy.
• Develops and communicates a plan for continuity of care and progression of referral.
• Performs written and oral reports to attending physicians and transplant leaders as appropriate.
• Completes and reviews brain death documentation and donor records for accuracy.
• Maintains charting in hospital and OneLegacy EMR and ensures policy-compliant documentation.
