Job summary

Residential Home Health and Residential Hospice is seeking a Transitional Nurse Liaison in the Kansas City area. The role requires an active Registered Nurse license and at least two years of RN experience, including acute care or oncology; hospice experience is preferred.

Hours are Monday through Friday, 8 AM to 5 PM CST, totaling 40 hours per week. Beneficial skills include EPIC, HCHB, and Microsoft Office, with a driver’s license for approximately 20% travel to patient locations.

Qualifications

• Associate degree from an accredited School of Nursing.
• A minimum of two (2) years of experience working as a Registered Nurse including acute care or oncology experience, hospice experience preferred.
• Experience with Microsoft Office Suite, EPIC, and HCHB preferred.
• Able to transfer patients using proper body mechanics to promote safety for patient and staff.
• Proficient with computers.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Obtain complete demographic information and medical history including diagnosis and PCP information.
• Introduce the Agency to the patient/caregiver and coordinate start-of-care visit.
• Order DME, supplies, and medications related to hospice care.
• Deliver community education for assisted living facilities, SNFs, and hospitals.
• Provide patient education at bedside regarding hospice and end-of-life care.
• Attend discharge planning meeting with facility team to ensure safe discharge home.
• Determine hospice eligibility and appropriate level of care.
• Communicate with the clinical team on complex patients to prevent re-hospitalizations.

Job Description

Registered Nurse License is REQUIRED for this position.

Compensation: $80,000.00 - $91,000.00. Plus a first-year $10,000.00 first year success bonus, paid out quarterly.

The base compensation range for this role is fixed, with a maximum cap of $91,000.00. We want to be transparent about this as we continue discussions.

Hours: Monday - Friday, 8AM - 5PM, CST

What you’ll do in this role:

• Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es)
• Obtain complete/accurate demographic information and medical history, including diagnosis for care and primary care physician information
• Introduce the Agency to the patient/caregiver, explain scope of our services and skilled services requested, and coordinate start-of-care visit
• Order requested DME, supplies, and medications related to hospice care
• Deliver community education as requested for assisted living facilities, skilled nursing facilities, and hospitals
• Provide patient education at bedside regarding hospice, disease progression, and end-of-life care
• Attend discharge planning meeting as requested, working with the facility team to provide a safe and successful discharge home
• Determine hospice eligibility and appropriate level of care
• Communicate with the clinical team on all complex, chronically ill patients, assisting staff and referral sources to prevent re-hospitalizations

We are looking for compassionate Transitional Nurse Liaisons with:

• Associate degree from an accredited School of Nursing
• A minimum of two (2) years of experience working as a Registered Nurse including acute care or oncology experience, hospice experience preferred
• Experience with technologies such as Microsoft Office Suite, EPIC, and HCHB preferred
• Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety
• Ability to accept different lifestyles, cultures, beliefs, and values
• Proficiency with computers
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
