Job summary

Elara Caring in Ohio is seeking a dedicated Registered Nurse to deliver high-quality home health care. You'll assess, plan, and coordinate patient care across settings, ensuring compassion and safety in every visit.

You'll partner with the care team, educate families, and adapt plans as patient needs evolve, with travel between patient homes a core part of the role. A valid RN license and 1+ year experience are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.

Job Description

To continue to be an industry pioneer in delivering unparalleled care, we need a Registered Nurse with commitment and compassion. As a Registered Nurse, you’ll contribute to our success in the following ways:

• Assess patients’ physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.
• Complete initial evaluations and reassess nursing needs during follow-up visits based on patient, family, and facility needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Update the Plan of Care as patient needs change and coordinate revisions with the appropriate care team members.
• Educate and support patients, families, caregivers, and facility staff on the Plan of Care, hospice philosophy, and pain and symptom management.
• Collaborate with the Medical Director, attending physician, branch leadership, facility staff, and interdisciplinary care team to coordinate patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to handle multiple priorities, documentation requirements, and deadlines.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
