Job summary

Elara Caring is seeking a dedicated Hospice Registered Nurse Case Manager for a full-time role in Grand Rapids, MI. You will assess patient and family needs, monitor conditions, and communicate changes to the care team to ensure optimal comfort and quality of life.

You will educate patients and families on hospice care, pain control, and symptom management, coordinating with facilities and home settings. Travel between patient homes is required; a valid RN license and 1+ year of clinical

• Graduate of an accredited nursing program (Certificate, Diploma, AS, or BSN).
• 1+ year of clinical nursing experience.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Valid driver's license and ability to travel between patient homes.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly report changes to physician and care team.
• Educate and support patients, families, caregivers, and facility staff in the Plan of Care.
• Provide education on hospice philosophy, pain and symptom management to staff in care settings.
• Provide compassionate support at end-of-life and complete bereavement assessments.

Job Description: Hospice Registered Nurse Full-Time

To continue to be an industry pioneer in delivering unparalleled care, we need a Registered Nurse Case Manager with commitment and compassion. As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
