Job summary

Centurion Health seeks a full-time Transitional Healthcare Liaison for the Parole District in Bloomington, IN. The role develops and implements healthcare plans for inmates scheduled for release, coordinating with state agencies to ensure seamless pre-discharge planning and access to services.

Two years in correctional settings and re-entry experience are preferred; CPR/AED certification and TB screening are required.

Qualifications

• Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required
• Two (2) years of experience working in a correctional setting
• Experience with/knowledge of community and state programs and services
• Two (2) years of experience in re-entry services or release planning preferred
• Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening
• Must be appropriately and actively certified in CPR and AED use
• Must be able to pass background investigation and obtain agency security clearance where applicable

Responsibilities

• Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release
• Coordinate with Indiana Department of Correction and other agencies for pre-discharge planning
• Ensure re-entry services and resources are provided to individuals re-entering society
• Occasional travel to juvenile facilities within Indiana for visits

We are seeking a full-timeTransitional Healthcare Liaisonfor needs at theParole Districtlocated inBloomington, Indiana.

Pay Rate $25 per hour + benefits!

The Transitional Healthcare Liaison will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Liaison will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources. Short term travel will be required to visit other juvenile facilities within the state of Indiana.
