Job summary

Qualifications

• Bachelor’s degree in Social Work/Human Services preferred.
• Minimum of two years of professional experience with clients who have behavioral health needs.

Responsibilities

• Provide case management and crisis intervention services to residents.
• Coordinate access to mental health, substance use, medical, vocational, and other supportive services to promote housing stability and recovery.
• Collaborate with residents, service providers, and community partners to support successful community integration.
