Job summary

Piedmont Atlanta Hospital in Atlanta, GA seeks a Social Work professional to promote patient and family wellness, improve care outcomes, and develop comprehensive transitional care plans for patients with complex needs in inpatient and ED settings.

You will collaborate with physicians, care teams, external agencies and families to ensure high-quality care and effective transitions. Masters in Social Work is required; 1 year of acute/post-acute experience is expected; LMSW/ACM/CCM are preferred.

Qualifications

• Masters in Social Work required.
• 1 year experience in an acute or post-acute setting required.
• LMSW, ACM, and CCM are preferred certifications.

Responsibilities

• Promote patient and family wellness and improve care outcomes through transitional care planning.
• Collaborate with physicians, care teams, external agencies and families to support quality care and effective transitions.
