Job summary

Qualifications

• 1 year of experience in an acute or post-acute setting.
• LMSW - Licensed Masters Social Work Preferred
• Accredited Case Manager (ACM) Preferred
• CCM - Certified Case Manager Preferred

Responsibilities

• Promote patient and family wellness and improved care outcomes.
• Develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and ED environments.
• Collaborate with service team members, physicians, patients, external agencies and families to ensure quality transitions.

Education

Master's Degree in Social Work from an accredited social work program
