Job summary

Qualifications

• Bachelor's degree in Nursing from an accredited program.
• RN license to practice in Texas; Basic Life Support required.
• 3 years RN experience in the relevant area; 1 year in Case Management/Quality/Coding preferred.
• 1 year of leadership/clinical experience preferred.
• Strong communication skills; proficient with MS Word.

Responsibilities

• Coordinate transitional care between patients, families, physicians and care teams.
• Plan and implement short- and long-term care strategies to meet patient needs across the continuum of care.
• Promote quality improvement initiatives and model ServiceFIRST behaviors in patient and staff interactions.
