Job summary

Wellstar Health System in Georgia seeks a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure care needs are met.

The CC RN plans effectively to meet patient needs, manages length of stay, and promotes efficient utilization of resources through care progression, facilitation, and discharge planning with the interdisciplinary team.

Qualifications

• RN licensure required upon hire in Georgia
• Minimum 1 year nursing experience in an acute care setting
• Associates degree in Nursing or Diploma; BSN preferred for progression

Responsibilities

• Initiates assessment of patients' chronic disease management needs, psychosocial risk factors, and resource availability.
• Manages all aspects of discharge planning, coordinates timely discharge and post-acute referrals as needed.
• Facilitates care progression by communicating with physicians and care team to resolve delays and barriers.
• Completes initial clinical/psychosocial assessment and documents interactions in the medical record.
• Provides mentorship and participates in mandatory education and department goals.
