Job summary

Wellstar Health System is seeking an RN Care Coordinator with hospital experience to assess transitional care needs and coordinate across the continuum. You will engage patients and families to ensure care needs are met, plan discharge, and promote efficient use of resources.

Responsibilities include collaborating with physicians, care team, and social work to manage barriers, document care progress, and participate in rounds for timely transitions.

Qualifications

• Associates or Diploma in Nursing is required.
• Bachelor’s in Nursing preferred.
• RN licensure with state or compact license.
• 1 year nursing experience in acute care.
• Strong communication and care coordination abilities.

Responsibilities

• Assess patients’ transitional care needs and coordinate across care continuum.
• Collaborate with care team to establish discharge plans.
• Manage discharge planning and ensure timely care transitions.
• Document assessments and progress notes clearly and timely.
• Identify barriers to discharge and engage stakeholders for resolution.
