Alaska Native Tribal Health Consortium (ANTHC) is seeking a Translational Care Manager (RN) in Anchorage, Alaska to manage proactive care planning and transitions. This role collaborates with medical providers, patients and families to promote wellness and satisfaction across the continuum of care.

Formación

• Bachelor's Degree in Nursing required.
• Non-supervisory professional RN experience preferred.
• Current RN license in Alaska and BLS.
• Experience in care coordination or case management is preferred.

Responsabilidades

• Identify care management patients within 24 hours of ED or inpatient referral.
• Develop and coordinate discharge plans for high-risk patients with their families and care team.
• Review patient progress against discharge goals and adjust plans as needed.
• Coordinate with pre-authorization and financial counselors for post-acute care.
• Communicate patient needs with the site coordinator and multidisciplinary team.
• Support Transitional Care Program development and quality improvement efforts.
