Alaska Native Tribal Health Consortium seeks a front-line Manager to oversee the Continuum of Care Management Program at ANMC. You will supervise discharge planning, transition of care, and social work, ensuring effective patient flow and compliance with Medicare and regulatory standards.

Collaborate with nursing leadership, physicians, and case management to optimize staffing and care coordination across the state.

Qualifikationen

• Bachelor’s Degree in social work or nursing. Progressively responsible work experience may be substituted for nursing education on a year by year basis.

Aufgaben

• Discharge planning and transition of care management for RN/SW caseloads; ensure coordinated discharge plans with patient, family and healthcare team.
• Supervise COC discharge planners, transition of care and social workers; act as content specialist.
• Ensure compliance with Medicare Conditions of Participation and ANMC policies; align with Joint Commission standards.
• Lead staffing decisions, orientation for new employees, and daily workload distribution.
• Develop and implement policies and protocols for discharge coordination and transition of care.
• Promote quality outcomes, patient satisfaction, and cost-effective care delivery.
