Qualifications

• Doctor of Pharmacy degree required.
• Advanced training (PGY-2) or equivalent and current board certification.
• NYS license with eligibility for allied health credential.
• Experience in ambulatory, hospital, or transitional care settings.
• Bilingual language skills preferred.

Responsibilities

• Lead pharmacotherapy management for high-risk patients during hospitalization and post-discharge.
• Titrate GDMT under CDTM agreements for heart failure, ASCVD, diabetes, COPD, CKD.
• Identify opportunities to prevent readmissions and support transitions.
• Address access, affordability, adherence, and social needs related to medications.
• Serve as a liaison across inpatient, ambulatory, and post-acute care settings.
• Collaborate with physicians, nurses, and care teams to design high-reliability care pathways.
• Contribute to quality-improvement initiatives aligned with CMS Star Ratings and HEDIS.
• Engage in telehealth, RPM workflows, and research to scale care pathways.
• Participate in academic activities and dissemination of outcomes.
