Qualificações

• Doctor of Pharmacy degree required.
• Advanced residency training or equivalent required; board certification preferred.
• NYS license with eligibility for allied health credential required.
• Board certification (BCPS/BCACP or equivalent) preferred or to be obtained within 6 months.

Responsabilidades

• Leads longitudinal pharmacotherapy management for a defined panel of high-risk patients during hospitalization and post-discharge.
• Initiates, optimizes, and titrates guideline-directed medical therapy under CDTM agreements for heart failure, ASCVD, diabetes, obesity, CKD, COPD, asthma, and pneumonia.
• Identifies high-impact clinical opportunities early post-discharge to prevent readmissions.
• Addresses medication access, affordability, adherence, and health-related social needs.
• Acts as a clinical bridge across inpatient, ambulatory, and post-acute settings to ensure seamless care transitions.
• Partners with physicians, advanced practice providers, nurses, and navigators to design and implement high-reliability care pathways.
• Engages with post-acute partners such as home care, SNFs, and rehabs; contributes to quality improvement initiatives.
• Participates in program development, telehealth optimization, RPM workflows, and scaling disease-specific pathways.
• Involves academic and scholarly activities including research and dissemination of outcomes.

Formação académica

Doctor of Pharmacy degree

PGY-2 residency in cardiology or ambulatory care or PGY-1 with board certification and 3 years of relevant experience

NYS license with eligibility to be credentialed as an allied health professional

Active Board of Pharmacy Specialties certification or completion within 6 months of hire
