Job summary

Montefiore Medical Center in the Bronx is seeking a Clinical Pharmacy Manager for Transitional Care Excellence. The role leads pharmacotherapy for high-risk patients across inpatient and post-discharge settings and utilizes telemedicine and RPM to optimize care.

Qualifications

• Doctor of Pharmacy degree required.
• Board certification (BCPS/BCACP) preferred or willing to obtain within the required timeframe.
• Direct patient care experience in ambulatory, hospital, or transitional care settings.
• Strong clinical judgment, communication, and interdisciplinary collaboration skills.
• NYS license eligibility to practice in NYS.

Responsibilities

• Leads pharmacotherapy management for high-risk patients during hospitalization and post-discharge.
• Initiates and titrates GDMT under CDTM with focus on HF, ASCVD, diabetes, obesity, CKD, COPD, asthma, and pneumonia.
• Identifies and acts on high-impact clinical opportunities post-discharge to prevent readmissions.
• Solves complex medication-related challenges including access, affordability, and adherence.
• Creates a clinical bridge across inpatient, ambulatory, and post-acute settings and partners with care teams.
