Job summary

Singing River Health System in Pascagoula, MS seeks a Transitional Care Management Nurse (RN) for a full-time, Monday–Friday 8:00 AM–4:30 PM role to bridge hospital teams and patients before discharge. You will assist with appointment scheduling, complete portions of the TCM phone process, and coordinate care to reduce readmissions, improve HCAHPS, and optimize discharge outcomes.

Requires RN license in Mississippi, 2+ years direct patient care, and MS Board of Nursing registration; case

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Mississippi RN license in good standing.
• Must be currently registered with the Mississippi State Board of Nurse Examiners.
• 2+ years direct patient care or clinic experience; case management experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Serve as intermediary between medical teams and patients to coordinate care across SRHS campuses.
• Assist with scheduling patient appointments before discharge with high accuracy.
• Provide applicable E&M codes to providers based on insurance coverage.
• Support efforts to reduce readmissions and improve discharge outcomes.
