Job summary

Qualifications

• Must graduate from an accredited School of Nursing.
• Currently licensed to practice in the State of Mississippi without encumbrances.
• Must be registered with the Mississippi State Board of Nurse Examiners.
• 2+ years direct patient care or clinic experience; Case Management preferred.

Responsibilities

• Serve as intermediary between medical teams and patients to coordinate TCM tasks.
• Assist with scheduling and appointment timing before discharge with a goal of 95%.
• Provide applicable E&M codes to providers based on insurance coverage.
• Support efforts to reduce readmissions, improve HCAHPS, and minimize LOS.
