Job summary

Renown Health is seeking a Transitional Care Coordinator to pre-screen patients for admission to rehabilitation or related services. You will collaborate with the medical director and interdisciplinary team to determine appropriate care pathways in accordance with CMS guidelines.

The role requires strong clinical experience, case management skills, and the ability to communicate effectively with physicians, patients, and families. Licensing in Nevada and CPR/BLS certification are required.

Qualifications

• Minimum of 5 years of clinical experience in acute care or rehabilitation settings.
• Experience with physician and community referral development preferred.
• Must be proficient in English (reading/writing/speaking).
• Ability to obtain Nevada licensure as LPN/RN/PT/SLP/OT/MSW is required or can be maintained.

Responsibilities

• Pre-screen patients for appropriateness of admission to Renown Rehabilitation Hospital or related services.
• Collaborate with the interdisciplinary team to evaluate patient needs and discharge plans.
• Communicate findings to accepting physicians and care coordinators.
• Maintain documents consistent with Renown Health policies and CMS guidelines.
