Job summary

Salinas Valley Health invites applications for a Transitional Care Coordinator. The role focuses on smooth discharge processes, post-discharge follow-ups, and optimizing patient outcomes through a comprehensive Continuum of Care plan.

You will work with physicians, caregivers, and multidisciplinary teams to ensure safe and effective transitions. The position requires an Associate Degree in Nursing, CA RN license, BLS, and 3 years of nursing experience.

Qualifications

• Current California RN license (BLS required).
• Associate Degree in Nursing is required; BSN preferred.
• 3+ years nursing experience and knowledge of community resources/case management.

Responsibilities

• Assess and plan care for discharged patients to reduce readmissions.
• Coordinate with physicians, hospital teams, and family to implement Continuum of Care plans.
• Follow up with patients 24–48 hours after discharge; Virtual/Home program calls as needed.
• Educate patients and families on medications, resources, and red flags.
• Document all interactions in the electronic health record.
