Job summary

IHA is seeking a Registered Nurse Care Manager in Michigan to coordinate care for high-risk members in a Grand Rapids hospital setting during day shifts. The role emphasizes assessment, care planning, and collaboration with the care team to minimize inpatient re-admissions.

Requirements include a BSN, Michigan RN license, BLS, and a valid Michigan driver’s license with at least three years of acute care or case management experience. Utilization/HMO experience preferred.

Qualifications

• Graduation from an accredited BSN program.
• Current Michigan RN license.
• BLS certification.
• Valid Michigan Driver’s license.
• Three years acute care or case management experience.
• Preferred: two years of utilization or HMO experience.

Responsibilities

• Keeps member/family informed and requests further assistance when needed.
• Functions as liaison between administration, members, physicians and other healthcare providers.
• Performs a Health Risk Assessment (HRA) and determines an acuity score for follow-up.
• Develops and implements outcome-based care plans with member input.
• Documents assessments, care plans, and related charting per policy.
• Initiates onsite hospital visits/rounds to assess patient progress.
