Job summary

MGA Homecare in Raleigh, NC seeks a Transitional Care Manager (RN) to coordinate private duty nursing efforts and strengthen partnerships with local healthcare providers. Qualifications

• Active RN license in good standing.

Responsibilities

• Develop relationships with pediatricians, hospitals, case managers, discharge planners, clinics, and other referral partners in Raleigh area.
• Identify opportunities to expand private duty nursing services and connect more families with in-home care.
• Educate healthcare providers and families about MGA’s private duty nursing services and benefits of one-on-one care.
• Use clinical knowledge to assess patient needs and identify appropriate homecare opportunities.
• Conduct in-person visits, presentations, networking, and community outreach to strengthen referral partnerships.
• Collaborate with clinical and operations teams to support transitions from referral to patient admission.
• Maintain relationships with referral partners, track outreach and referral activity, and identify opportunities for continued growth.
• Represent MGA Homecare professionally within the Raleigh healthcare community.

What We’re Looking For

• Active Registered Nurse (RN) license in good standing required.
• Clinical nursing experience preferred, especially pediatric nursing, private duty nursing, home health, or care coordination.
• Understanding of medically complex patients and the needs of families managing care at home.
• Strong communication, networking, and relationship-building skills.
• Interest in healthcare partnerships, referral development, and expanding access to home-based care.
• Self-motivated, organized, and comfortable working independently.
• Reliable transportation and willingness to travel throughout the Raleigh area.
