Job summary

Your Health is seeking a Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home across the Savannah area. You will perform home and facility visits, build trust with patients and families, and arrange home health and follow-up care with our providers.

The role requires travel between offices and care sites, daily reliability with transportation, and full-time schedule (8am-5pm).

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
• Certifications or licenses must be in good standing.
• Three years clinical experience preferred and ability to supervise staff.

Responsibilities

• Visit referrals and established patients in assigned facilities.
• Establish trust with patients to aid informed decisions during stays.
• Coordinate care based on the care team recommendations.
• Arrange services for home return (home health, rehab, follow-up).
• Schedule visits with primary care, virtual providers, specialists, pharmacists, social workers.
• Facilitate communication among healthcare professionals involved in care.
