Job summary

Your Health Organization seeks a dedicated Transitional Care Professional to coordinate patient transitions from hospitals and facilities to home in the Savannah area. You will conduct home and facility visits, build trust with patients, and coordinate post-acute services with the care team.

Responsibilities include scheduling visits, facilitating communication between providers, and ensuring adherence to the care plan while delivering compassionate, patient-centered service.

Qualifications

• Certified or licensed clinician in good standing.
• Min. 3 years clinical experience preferred.
• Public health or home health background is a plus.
• Ability to supervise and direct both professional and admin staff.
• Strong reading, writing, and verbal communication skills.
• Able to multitask and manage time effectively.

Responsibilities

• Visit referrals and established patients in assigned facilities.
• Establish trust with patients to support informed health decisions.
• Coordinate home health, outpatient rehab, and follow-up care.
• Schedule essential visits with PCP, specialists, and care team.
• Facilitate effective communication among health care professionals.
• Ensure adherence to the plan of care by patients and families.
• Interact with patients via phone, email, or portal to gather info.
• Provide compassionate, patient-centered service and address concerns promptly.
• Perform general admin tasks and support staff as needed.
• Utilize software systems to enhance patient care and efficiency.
