Job summary

Pardee UNC Health Care is seeking a Transitional Care Case Manager (TCCM) to coordinate care for high-risk patients transitioning from hospital to next level of care. This hybrid, full-time role supports up to 30 days after discharge and up to 90 days for specified procedures, ensuring continuity and patient education.

You will work within Acute Care Case Management to streamline plans of care, reduce readmissions, and promote patient autonomy through advocacy and communication.

Qualifications

• Must be licensed to practice as a Registered Nurse in North Carolina or a compact state.
• Basic Life Support (BLS) certification is required.
• Experience in care management or discharge planning is preferred.

Responsibilities

• Identify high-risk admissions and coordinate post-discharge plans.
• Review discharge plans and arrange follow-up with primary care providers.
• Educate patients and families on disease processes and medications as needed.
• Document activities in established care management software and maintain case records.
