Job summary

Pardee UNC Health Care in North Carolina is seeking a Transitional Care Case Manager to coordinate care transitions for high-risk patients leaving the hospital and prevent readmissions. You will follow patients for up to 30 days (90 days for total knee/hip replacements), perform assessments, plan care, and provide education to patients and families using evidence-based care coordination practices.

A current RN license in NC or a compact state and BLS are required; a Care Management certification

Qualifications

• Must be licensed to practice as a Registered Nurse in North Carolina or a compact state.
• Basic Life Support (BLS) certification.
• Care Management certification is expected after hire.
• Experience in Case Management/Discharge Planning preferred.

Responsibilities

• Identify high-risk admissions and prioritize patients; coordinate care across the healthcare continuum.
• Review discharge plans and arrange follow-up with the primary care physician; assess transportation needs as needed.
• Document activities, events, and information in established software systems; maintain updated case records.
• Collaborate with healthcare teams to support patient self-management and ensure privacy and safety.
