St. Helens High School in St. Helens, Oregon is hiring a Transition Specialist to assist students with disabilities in transitioning from school to employment under IDEA/504. The role is full-time (7.5 hours/day) with an hourly salary range of $20.64 to $25.69.

Responsibilities include case management, 1:1 instruction, collaboration with VR, IEP/504 planning, data collection, and student support across school and community settings.

Qualifications

• Two years of post-secondary education or an Associate's Degree, preferably with coursework in working with students in an educational setting.
• In lieu of the first item, must possess both a High School Diploma or equivalent and a qualifying assessment measuring reading, writing, and math.
• Knowledge/experience with IEP processes and activities.

Responsabilités

• Provide 1:1 and small group instruction in functional living skills related to employment.
• Act as liaison with VR and IEP meetings and coordinate transition services.
• Maintain student records and assist with data collection and reporting.
