Responsibilities

• Provide leadership and technical assistance to schools in developing, implementing, monitoring, and revising transition plans.
• Support staff in conducting age-appropriate transition assessments to identify students' strengths, interests, needs, and goals.
• Assist school teams in developing measurable post-secondary goals related to education, training, employment, and independent living.
• Design and deliver professional development for staff regarding effective transition planning and services.
