Kids for the Future in Hillsborough, NC is seeking a Transition Specialist to help high school students with disabilities prepare for life after graduation by delivering Pre-Employment Transition Services (Pre-ETS) in schools, communities, and workplaces.

You will mentor students, coordinate job shadowing and internships, document progress, travel between locations, and collaborate with families, schools, employers, and rehabilitation staff to support employment and independence.

Qualifications

  • Requires a 4-year degree (Associate's or Bachelor's) and ability to work independently with limited supervision.
  • Proficient computer skills including MS Office and Google Workspace.
  • Excellent organizational and communication abilities, both written and verbal.

Responsibilities

  • Provide Pre-Employment Transition Services (Pre-ETS) to students with disabilities in school, community, and workplace settings.
  • Deliver engaging instruction on career exploration, workplace readiness, self-advocacy, and postsecondary planning.
  • Plan, coordinate, supervise, and support students in job shadowing, internships, and work-based learning.
  • Assist students in developing workplace skills and professional behaviors for employment and education.
  • Build relationships with school personnel, parents/guardians, employers, and community partners to support outcomes.
  • Monitor progress, provide guidance, and maintain documentation and program reports.

Description
Primary Duties and Responsibilities
  • Provide Pre-Employment Transition Services (Pre-ETS) to students with disabilities in school, community, and workplace settings through individual, small-group, and large-group instruction.
  • Deliver engaging instruction and activities related to career exploration, workplace readiness, self-advocacy, postsecondary education planning, and employment preparation.
  • Plan, coordinate, supervise, and support students participating in job shadowing experiences, internships, work-based learning opportunities, and community-based activities.
  • Assist students in developing workplace skills, professional behaviors, and strategies for success in employment and educational settings.
  • Build and maintain positive relationships with school personnel, parents/guardians, employers, community partners, and vocational rehabilitation staff to support student outcomes.
  • Monitor student progress and provide guidance, feedback, and support to help students achieve individual goals.
  • Complete and maintain accurate service documentation, attendance records, case notes, and program reports in accordance with agency and funding requirements.
  • Participate in meetings, trainings , and professional development opportunities as assigned.
  • Travel between schools, OE offices, community locations, and employer worksites as needed to provide services and support students.
Knowledge, Skills, and Abilities
  • Ability to problem-solve, adapt to changing priorities, and exercise sound judgment.
  • Exercises sound judgment and initiative by anticipating needs, following through on responsibilities, and taking appropriate action with minimal supervision.
Requirements
  • Associate's or Bachelor's degree required .
  • Ability to work independently with limited on-site supervision while maintaining strong communication with supervisors and team members.
  • Strong computer skills, including proficiency with Microsoft Office and Google Workspace.
  • Excellent organizational, interpersonal, and communication skills, both written and verbal.
  • Ability to maintain accurate documentation and manage multiple priorities effectively.
  • Must successfully pass a background check and driving record review.
  • Verification of required immunizations or willingness to obtain required immunizations.
Summary

The Transition Specialist helps high school students with disabilities prepare for successful lives after graduation.
