Job summary

MercyOne in Des Moines, IA, is seeking a Clinical Pharmacist to optimize pharmacotherapy, conduct daily reviews, and support interdisciplinary care across designated units. You will participate in emergencies and rounding, ensuring safe, evidence-based medication use in a dynamic hospital setting.

Candidates must hold a pharmacy degree from an ACPE accredited school and have or be eligible for Iowa licensure; residency training (PGY-1 and/or PGY-2) or equivalent hospital experience is preferred.

Qualifications

  • Pharmacy degree from ACPE approved college.
  • ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience.
  • PGY-2 residency or 3 years of experience in the relevant clinical practice area preferred.
  • BLS certification within 3 months of hire.
  • ACLS and/or PALS within 3 months of hire; licensed or eligible for licensure in Iowa.
  • BCPS or specialty board certification within 2 years of hire or 10 years of experience.
  • Mandatory Reporter abuse training completion within 3 months of hire.

Responsibilities

  • Assess patient data to identify medication-related problems via profile reviews and rounding.
  • Participate in medical emergencies and formal alerts (trauma, stroke, sepsis, STEMI).
  • Review and interpret orders to ensure appropriate drug, form, and dose for age/weight/comorbidities.
  • Consult with providers and care teams to optimize pharmacotherapy for outcomes.
  • Provide drug information responses and uphold medication-use policies.
  • Serve as preceptor for pharmacy students, residents, and technicians.
  • Assist nursing with medication administration per policy.
  • Document care activities in the EMR per guidelines.
  • Identify and report medication errors and pursue process improvements.
  • Maintain awareness of laws, formulary, and practice guidelines.

Job description

Description: This position is eligible for relocation depending on location.

Clinical pharmacists are responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine within interdisciplinary healthcare teams . As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designated area (s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine . They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community .

Clinical pharmacists will be assigned area(s) of coverage based on experience, training, and competency . Rotate day and evening shifts; every third weekend .

  • Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
  • Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required .
  • Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary.
  • As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
  • Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
  • Serves as preceptor for pharmacy students/residents and assists with orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists and other health professionals.
  • Assists nursing with medication administration (oral, injectable, continuous infusion titration) depending on the work assignments and in accordance with regulatory and hospital policies.
  • Documents patient care activities including therapy plan, monitoring and follow-up in electronic medical record as per departmental guidelines.
  • Responsible for the completion of all assigned work during the shift, while maintaining the highest standards of quality and safety through the completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensuring appropriate documentation .
  • Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures, formulary and pertinent clinical practice guidelines while providing pharmaceutical care to patients.

Schedule: Full Time Rotating Shift

General Requirements
  • Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.
  • Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience required .
  • Successful completion of PGY-2 residency or 3 years of experience in the relevant clinical practice area preferred.
  • BLS certification within 3 months of hire.
  • ACLS and/or PALS certification within 3 months of hire based on practice area(s). Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.
  • BPS Board Certification (BCPS or specialty board certification in the relevant clinical practice area) within 2 years of hire or 10 years of experience in the relevant clinical practice area.
  • Other certifications if requested by direct supervisors/managers .
  • Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
