Job summary

Trinity Health is hiring a part-time pharmacist for the evening shift to deliver patient-focused, outcome-oriented pharmaceutical care across all ages. Review orders for dosing accuracy by age/weight, and coordinate with care teams to optimize therapy.

The role includes supervising technicians and students, ensuring quality and safety through checks and documentation, and responding to drug information inquiries for staff and patients. Licensing in Iowa is required or eligible.

Qualifications

• Pharmacy degree from ACPE-accredited college.
• Licensed or eligible for Iowa pharmacist licensure; must obtain license within 3 months.
• BLS certification within 3 months of hire.
• Mandatory Reporter abuse training within 3 months of hire.
• Ability to work evening shift in a part-time capacity.

Responsibilities

• Delivers medications to care areas in a timely manner and ensures secure storage.
• Collaborates with healthcare team to optimize pharmacotherapy and patient outcomes.
• Supervises and mentors technicians, interns, and students on shift.
• Maintains quality and safety via quality control checks and incident reporting.
• Provides timely drug information to healthcare professionals and patients.
• Participates in orientation and training of pharmacy staff and students.
• Adheres to laws, policies, formulary, and guidelines; maintains confidentiality.
• Provides professional customer service to patients and providers.
• Performs other duties as directed by the pharmacy manager.

Education

Pharmacy degree (BS or Pharm.D.) from ACPE approved college of pharmacy

Iowa pharmacist license or eligible; license within 3 months of hire

BLS certification within 3 months of hire

Mandatory Reporter abuse training within 3 months of hire

Description:

Delivers patient focused, outcome oriented pharmaceutical care to patients of all ages, including neonatal, infant, pediatric, adolescent, adult, and geriatric. Promote a positive work environment and interpersonal relationships, while maintaining the highest level of professionalism and confidentiality.

Reviews and interprets physician orders to assure that the drug, dosage form, and dose is appropriate for the patient’s age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary.

• Oversees and/or participates in assuring appropriate medications are prepared and delivered to care areas in a timely manner, and are stored both securely and in a manner appropriate for both stability and effective workflow.
• Works collaboratively with other healthcare team members to provide pharmacotherapy recommendations with intent to achieve optimal patient outcomes across the continuum of care.
• Supervises and mentors technicians, interns, and students working during their shift in order to maintain the accuracy and quality of pharmaceutical services.
• Maintains quality and safety in the department through completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensures appropriate documentation.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and MercyOne medication use policies to assure safe and appropriate use of medications.
• Participates in the orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists and other health professionals.
• Knowledgeable and adheres to State and Federal laws, regulations, pharmacy policies, procedures and standard operating procedures, formulary and pertinent clinical practice guidelines.
• Provides professional, engaging customer service to all patients, and healthcare providers with whom they work.
• Performs other job functions as directed by the pharmacy manager/supervisor.

Minimum Qualifications:

• Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.
• Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire. 2. BLS certification within 3 months of hire. 3. Other certification if requested by direct supervisors/managers. 4. Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
