Job summary

IHA in Des Moines, IA is seeking a Clinical Pharmacist to join our decentralized team. This full-time role focuses on optimizing pharmacotherapy, ensuring safe patient care, and advocating evidence-based medicine within interdisciplinary healthcare teams.

You will conduct daily medication therapy reviews, consults, and other clinical services for patients across med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine.

Qualifications

• Pharmacy degree from an ACPE-approved college of pharmacy.
• ASHP-accredited PGY-1 Residency or 3 years hospital experience.
• PGY-2 residency or 3 years experience in the relevant clinical practice area preferred.
• BLS certification within 3 months of hire.
• ACLS and/or PALS certification within 3 months of hire based on practice area.
• Licensed, or eligible for licensure, pharmacist in Iowa within 3 months of hire.
• BCPS or specialty board certification within 2 years or 10 years of experience in the relevant clinical practice area.

Responsibilities

• Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
• Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patients age, weight, and comorbidities.
• Consult with providers, nurses and other health professionals involved in patient care regarding pharmacotherapy optimization to achieve optimal patient outcomes.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information and facility medication use policies.
• Serves as a preceptor for pharmacy students/residents and assists with orientation and training.
• Assists nursing with medication administration (oral, injectable, continuous infusion titration).
• Documents patient care activities in the electronic medical record.
• Maintains awareness of laws, regulations, policies and formulary to ensure safe use of medications.

They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community.

As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designated area(s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine. They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community.

Clinical pharmacists will be assigned area(s) of coverage based on experience, training, and competency. Rotate day and evening shifts; every third weekend.

• Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
• Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
• Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient\'s age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary.
• As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
• Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
• Serves as preceptor for pharmacy students/residents and assists with orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists and other health professionals.
• Assists nursing with medication administration (oral, injectable, continuous infusion titration) depending on the work assignments and in accordance with regulatory and hospital policies.
• Documents patient care activities including therapy plan, monitoring and follow-up in electronic medical record as per departmental guidelines.
• Responsible for the completion of all assigned work during the shift, while maintaining the highest standards of quality and safety through the completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensuring appropriate documentation.
• Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures, formulary and pertinent clinical practice guidelines while providing pharmaceutical care to patients.

General Requirements

• Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience required.
• Successful completion of PGY-2 residency or 3 years of experience in the relevant clinical practice area preferred.
• BLS certification within 3 months of hire.
• ACLS and/or PALS certification within 3 months of hire based on practice area(s). Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.
• BPS Board Certification (BCPS or specialty board certification in the relevant clinical practice area) within 2 years of hire or 10 years of experience in the relevant clinical practice area.
• Other certifications if requested by direct supervisors/managers.
• Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
