IHA in Des Moines, IA is seeking a Clinical Pharmacist to join our decentralized team. This full-time role focuses on optimizing pharmacotherapy, ensuring safe patient care, and advocating evidence-based medicine within interdisciplinary healthcare teams.
You will conduct daily medication therapy reviews, consults, and other clinical services for patients across med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine.
This position is eligible for relocation depending on location. Clinical pharmacists are responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine within interdisciplinary healthcare teams. As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designated area(s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine. They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community.

Clinical pharmacists will be assigned area(s) of coverage based on experience, training, and competency. Rotate day and evening shifts; every third weekend.
