Qualifications
- 2 years Licensed Vocational Nurse (LVN) nursing experience.
- Active and unrestricted TX Compact License.
- Resides within Texas.
Responsibilities
- Coordinate care for members experiencing health status changes.
- Ensure seamless transition to next care setting.
- Complete medication reconciliation as necessary.
- Engage with members to address health and social barriers.
- Facilitate Interdisciplinary Care Team Meetings.
