Molina Healthcare of California is seeking a Transition of Care Coach LVN/LPN. This remote role requires an active California LVN/LPN license. Typical hours are Monday–Friday, 8am–5pm Pacific Time, with 40–50% local travel as needed.

Qualificações

• Must be an LVN/LPN with active California licensure.
• Experience in hospital discharge planning or care management is required.
• Proficient in MS Outlook/Excel/Word; able to learn Windows-based software.

Responsabilidades

• Supports care transition activities and coordinates discharge from hospital to other settings.
• Oversees a 30-day transition program to reduce readmissions.
• Collaborates with discharge planners, hospitalists, providers, and families.
• Ensures safe transitions with adequate caregiving and medical oversight.
• Coordinates with providers and community resources for necessary services.
• Conducts face-to-face hospital visits and post-discharge visits for high-risk members.
• Educates members on medication management, follow-up care, and home-based services.
• Utilizes Coleman Transition model and Molina guidelines to support change.
• Facilitates ICT meetings and may provide consultative resources to peers.
• Travel 40-50% may be required depending on state/contractual needs.
