TriWest Healthcare Alliance seeks a Transition of Care Coordinator to oversee care transitions for TRICARE beneficiaries. You will coordinate assessments, engage in discharge planning, and perform post-discharge follow-up to ensure continuity of care and safety.

The role requires an RN/LSW/LCSW with a current license, multi-state licensure, U.S. citizenship, and ability to obtain DoD background clearance. Strong communication and organizational skills are essential.

Qualifikationen

• Required licensure in appropriate state or territory and current unrestricted license.
• Multistate compact license is required.
• U.S. Citizenship is required for DoD position.
• 2+ years in Care Coordination or Case Management.

Aufgaben

• Coordinate transition of care and assist with discharge planning.
• Conduct post-discharge follow up and assess needs for high-risk patients.
• Review authorizations and referrals per policy and ensure compliance.
• Document assessments and coordination activities in the MMS system.
• Escalate complex clinical or safety concerns as needed.
