Molina Healthcare is hiring a licensed behavioral health professional to serve as Transition of Care Coordinator for Medicaid members recently discharged from psychiatric hospitals. This hybrid role combines remote work from a home office with local hospital visits to ensure seamless service access and reduce readmissions.

Applicants must reside in Thurston or Mason County, WA and hold an active WA license among listed options. Mileage reimbursement is included; hours are Mon–Fri, 8 AM–5 PM PST.

Qualifikationen

• Licensed behavioral health clinician with active state license(s).
• Experience in hospital discharge planning or care management preferred.
• Knowledge of care transition models and community resources.

Aufgaben

• Supports care transition activities from hospital admission through 30 days post-discharge.
• Coordinates with discharge planners, hospitalists, outpatient providers and families.
• Ensures safe transitions with appropriate caregiving, medical oversight and equipment.
• Performs face-to-face and home visits as needed to monitor members.
• Leads interdisciplinary care team meetings and provides education to non-BH care managers.
• Travel 40-50% locally to meet members and discharge team.
