Job summary

Interim Healthcare Staffing in the West Metro is seeking a Transitional Nurse Liaison (RN) for a full-time, temporary assignment from November through March 2027. The TNL coordinates safe transitions of care across hospital post-acute networks and collaborates with care teams to ensure seamless discharge and follow-up. Requires two years of RN experience, MN RN license, and BLS.

Preference for BSN, with home care/hospice experience and OASIS familiarity. Location: western suburbs incl.

Qualifications

• Two years of RN experience required.
• Current MN Registered Nursing License required.
• BLS certification required and maintained.

Responsibilities

• Coordinate safe, timely transitions of care across hospital post-acute networks.
• Serve as primary clinical liaison among hospital teams, patients, and post-acute partners.
• Ensure information, medications, supplies, and documentation are ready for start of care.
