Job summary
Interim HealthCare Staffing in the West Metro is seeking a Transitional Nurse Liaison (RN) for a full-time, temporary assignment. The RN coordinates safe transitions of care within the HealthPartners post-acute network, acting as the primary clinical liaison among hospital teams, patients, and post-acute partners. Schedule: M-F 8am-4:30pm with weekend rotations every 6th/7th weekend.
Location includes Eden Prairie, Minnetonka, Hopkins, Mound, Chanhassen. Pay up to $74.20/hour plus mileage.
Qualifications
- RN with at least two years experience.
- Current MN Registered Nursing License.
- Current BLS certification for health professionals.
- Knowledge of age-related growth and development principles to ensure patient safety.
- Proficient in operating: PC, telephone, fax, copier, printer, and EMR.
Responsibilities
- Coordinate safe, timely transitions of care for hospital post-acute patients.
- Serve as primary clinical liaison among hospital care team, patients, caregivers, and post-acute partners.
- Coordinate referrals, education, and required documentation for transitioning patients.
- Ensure receiving team has medications, supplies, equipment, and communication to begin care.
