Transition Coordinator - Franklin and surrounding counties

CareStar, Inc. is seeking a Transition Coordinator in the Operations Department to assist individuals served with locating housing, coordinating the physical move to a community setting, and ensuring efficient utilization of program funds. The Transition Coordinator collaborates with long-term care (LTC) facilities, Case Managers, and individuals served to initiate services that meet individual needs in a cost-effective and service-efficient manner, respecting personal preferences, rights to self-determination, strengths, needs, and barriers.

THE POSITION

CareStar is looking for motivated individuals who:

• Act as a point-of-contact to assist individuals in living in their homes and/or community.
• Assist individuals with linkages to service providers and community resources.
• Identify barriers to compliance with healthcare regimes, appointments, and screenings.
• Work with individuals to create solutions to barriers and empower them to manage contacts, appointments, and screenings with healthcare professionals.
• Communicate and collaborate with the individual’s healthcare team, including social workers and waiver case managers.
• Are at least 18 years old.
• Have a High School Diploma or higher education.
• Have at least 12 months of experience delivering services in healthcare, behavioral health, or social services.
• Have experience advocating for individuals with chronic illnesses, mental health or substance use disorders, and physical disabilities.
• Adhere to the CareStar Rules and policies, maintaining confidentiality and following acceptable use policies for information systems.

This position is based in Franklin County and covers surrounding counties.
