CareStar, Inc. is seeking a Transition Coordinator in the Operations Department to assist individuals served with locating housing, coordinating the physical move to a community setting, and ensuring efficient utilization of program funds. The Transition Coordinator collaborates with long-term care (LTC) facilities, Case Managers, and individuals served to initiate services that meet individual needs in a cost-effective and service-efficient manner, respecting personal preferences, rights to self-determination, strengths, needs, and barriers.
