The Transition Coordinator coordinates day-to-day operations of transition to adulthood (employment, volunteerism, service, independent living, leadership, advocacy, and post‑secondary education) programs and services. This position works collaboratively with the executive coordinator for transition and provides guidance to the transition specialist in developing, implementing new programs, services and activities; maintaining existing programs, services and activities; monitoring person-centered plan (plan of care) development, service coordination/case management, and internships; and overseeing direct training and technical assistance to youth and young adults with disabilities, family members and those that serve them.
Bachelor’s degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling or related field. Three to five years of experience or training beyond a bachelor’s degree in working with persons with disabilities providing transition training, technical assistance and/or direct services.
Master’s degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling or related field. Social work licensure or case management certification.
