You will provide hands-on support, guidance, and referrals designed to connect clients to housing and essential services. The role emphasizes compassionate street-based outreach, collaboration with partner agencies, and documentation of client interactions to inform housing stability plans.

Qualifications

• Bachelor's degree or equivalent experience required.
• Strong communication, documentation, and interpersonal skills.
• Ability to work in the field with individuals experiencing homelessness.
• Willingness to travel across boroughs and work in transit environments.

Responsibilities

• Conduct outreach in transit locations with compassion and professionalism.
• Connect clients to housing, medical, mental health, and support services.
• Complete brief assessments, provide escorts, and make referrals as needed.
• Document outreach activities and client interactions for case records.
• Work primarily in high-need transit areas and collaborate with partner agencies.
• Coordinate with outreach teams to ensure client safety and service continuity.
